2. Enter your DOB and state Then enter site. 
3. Click create an account and after you do you will see the free item. Checkout. Choose the first shipping option, I think it will be faster. 
4. CALIFORNIA RESIDENTS READ HERE FIRST: If you select California as your state you are supposed to receive a beach mat instead but it seems to be out of stock now. You may want to try selecting a different state in the beginning instead of California so at least you can get a t-shirt (not sure if they will ship it, but worth a try)
