- Your entry will count from the date your Envelope is Postmarked and is valid for the entire contest. If you mail it out on 1/4, the start date, you will be entered for the each day of the contest. If you mail it out after that you will be counted from the date that you mail it out
- Hand print the following information on a 3″ x 5″ notecard or index card and place it in an envelope:
- your full name
- your e-mail address
- your mailing address
- your date of birth (mm/dd/yyyy)
- your home phone number;
- and your responses to the following questions: (write out the questions and your answers to them as well)
- What tax preparation method did you use to file your 2015 taxes? (Sample Answer: Electronic)
- What form did you use to file your 2015 taxes? (Sample answer: 1040)
- Have you used H&R Block to file your taxes? (Sample Answer: No)
- You will receive one entry into the Promotion based on the applicable Entry Date, which is the date your entry is postmarked. Mail it in an envelope with proper postage to:
